1280 x 720 pixels in a 16:9 aspect ratio. YouTube accepts any 16:9 image at least 640 pixels wide, but 1280 x 720 is the recommended resolution and scales down cleanly to every smaller placement.
Never. It either crops the longer edge from the centre to reach an exact 16:9 frame, or fits the whole image inside the frame with padding. The aspect ratio of your content is always kept.
There is little benefit. YouTube displays thumbnails at roughly 1280 pixels wide, so a 4K upload is downscaled and mainly costs file size. 1280 x 720 is the practical choice.
Most likely the source was smaller than the target, so it had to be enlarged. Start from an image at least 1280 pixels wide. If it is sharp in YouTube Studio but soft in the public feed, YouTube may still be processing it.
